For our new patients, please print out and complete the following forms before your first appointment.  If unable to access or print these forms prior to your appointment, then please arrive 15 min early to complete them in office.  Thank you!

What to bring with you on your first appointment:

3. Any available laboratory results, radiology/test reports, medical records and current                            _ __medication(s) and/or supplement(s)

**Please note that we currently accept cash and personal checks only. Our office does not accept credit cards at this time.**
