Membership-Based Personalized Care

Our membership model was created to strengthen the physician-patient relationship and provide a higher level of long-term, personalized care.

By maintaining a smaller, more focused practice, we are able to spend meaningful time with patients, offer thoughtful follow-up, and create individualized care plans designed around prevention, wellness, and chronic disease management.

This model is especially valuable for patients who benefit from continuity of care, regular monitoring, coordination of testing, lifestyle guidance, and ongoing physician access.

Why Membership Works

Membership allows us to provide:

• Longer, unrushed visits
• Direct physician communication
• Telemedicine follow-up when appropriate
• Reduced scheduling delays
• Personalized preventive care
• Chronic condition management
• Nutritional and lifestyle guidance
• In-office blood draws when available
• A more proactive approach to long-term health

Our goal is to deliver healthcare that is thoughtful, responsive, and tailored to your individual needs.

Becoming a Member

The decision to join membership is typically made after your initial evaluation process (at your second visit), allowing both doctor and patient to determine whether the practice is the right long-term fit.

Initial Visit

Your first visit is typically 60–120 minutes and includes a comprehensive review of your medical history, current concerns, medications and supplements, examination, and discussion of appropriate next steps.

This may include bloodwork or additional testing when indicated. Most standard tests are typically covered by insurance, though any potential out-of-pocket testing costs would be discussed in advance.

Follow-Up Visit

At your follow-up visit (second visit), Dr. DeVito reviews results in detail and develops a personalized plan that may include:

• Nutrition strategies
• Exercise recommendations
• Sleep optimization
• Stress management
• Medication adjustments
• Supplement recommendations when appropriate

Following this process, eligible patients may choose to enroll in membership.

Membership Fees & Availability

Membership capacity is intentionally limited in order to preserve physician access, personalized attention, and scheduling availability.

Current Membership Fees

• $85/month for the first household member
• $75/month for each additional household member

Membership fees are billed monthly by credit card through our secure PayPal payment processing service. A PayPal account is not required.

Flexible Commitment

We believe patients should remain members because they value the care they receive—not because of a long-term contract.

There is no long-term membership commitment. A 30-day notice is requested for cancellation.

If membership is discontinued, future re-enrollment requires a two-year waiting period, after which any request to return would be subject to then-current availability and placement on the active waitlist.

* Body Composition testing is conducted (if necessary per discussion with Dr. DeVito) at each in-office visit at no additional charge.  There is a $30 fee for a Body Composition test done  at a different time than an in-office visit.
